What you will need for this tutorial:
A Preset shape of your choice.
A nice fat font so the pattern will show.
Eye Candy 4000 Download here
Ulead Art Texture Download here
If you don't have Ulead you can also floodfill with either a gradient or a pattern of your choice. Great effects that way also.
If you want to make a preset shape of your own, here is a link that shows you how to do this.
OK now let's get started!!!
1. Open a new image 600x600 transparent background. Floodfill with white.
2. Set both foreground and background colors to black and draw your preset shape.

3. Go to your layer palette and select layer with your shape on it and Convert to Raster Layer. Selections/Select All and then Selections/Float.
4. Effects/Plugins/Ulead Effects/ArtTexture. When you get the following window, click on the Edit button.

5. For the one I have made I selected Palette-30. Then click OK.

6. Below is the effects you will get from this palette. If you click on the button next to the Edit button the pattern colors will reverse.

7. I chose to reverse mine. When you are happy with your choice click OK and your shape will be filled. Do Not Deselect.

8. Effects/Plugins/EyeCandy4000/Bevel Boss and use the following settings. Click OK.

9. Go to Selections and Select None. Now go to Effects/3D Effects/Drop Shadow and use these settings.

This is what you should have now.

10.If you choose to add a name it is done the same way as the first image. Add a new layer. Click on your text tool and choose the font you wish to use. Click "Vector" so you can then stretch your name to look the way you want it. When you are happy with the placement and size, go to your layer palette and convert this layer to a Raster Layer. Now repeat from Step 3 exept for the converting to Raster Layer as you have already done this.
11. Crop off any excess white you do not want. At this point you can go to your layer palette and activate layer 1 and change your background color to a pattern or different color or you can delete layer one and you will end with a transparent background which you can save as a psp for use later. Resize to whatever size you wish. Effects/Sharpen/Sharpen. Your All done!!! Hope you enjoyed this tutorial.

Here is another more colorful effect using the Ulead Effects/ArtTexture but using Palette 13 instead.
